What is Transitions?
Transitions offers volunteer services and practical support at no cost for residents of Santa Clara County who are coping with an advanced or serious illness.
Who Can Refer a Client to the Program?
Referrals can come from anyone; doctors, social workers, self-referral, family or friends.
What Services are Provided?
Transitions clients are supported with non-medical assistance such as:
- Accessing and coordinating community resource
- Understanding and review of Advance Healthcare Directive
- Monthly check-in phone calls
- Volunteer support
Who Are Our Volunteers and What Will They Do?
Our volunteers are your neighbors, called to serve the community. Before volunteers visit clients an extensive training program is completed with ongoing bi-monthly education.
Volunteer assistance may include:
- Running errands
- Food preparation
- Help with household organization
- Caregiver respite
Complementary services available:
- Pet companions
How Do I Sign Up?
Prior to acceptance into Transitions, a client assessment is completed by phone. This is followed by a visit with the program coordinator who discusses needs and goals with the client and their family or caregivers.
