The Transitions program utilizes dedicated and trained volunteers to provide assistance with:
- Accessing and coordinating community support services and resources
- Emotional and spiritual support
- Errands, chores, food preparation
- Transportation
- Breaks for the family, caregiver
- Friendship and companionship
- Complimentary services such as art therapy, massage, nail and hair care
- Dealing with feelings of confusion regarding complex decisions and emotional issues related to the life-limiting illness
- Provide guidance in discussing your condition with your doctor and other medical professionals
- Understanding Advanced Health Care Directives and how they apply to you
- Facilitating communication with you and your family regarding your illness
Who is eligible for Transitions?
Anyone facing a life-limiting illness with a prognosis of one year or less whether or not they are undergoing treatment should consider Transitions.
How do I access Transitions?
Anyone can make a referral to Transitions: the client, a loved one, or physician. Call 408 559-5610 for the Transitions Coordinator. Our Coordinator will discuss the program with the client, and then contact the physician to determine eligibility. Services are provided to the clients in their home, nursing home or assisted-living facility.
What does Transitions cost?
Transitions services are provided free of charge to eligible residents of Santa Clara County. Transitions is a program funded by community donations and grants.
Is Transitions part of hospice?
No. Transitions is a separate program. You do not have to consider hospice care to access Transitions.
What is the mission of Transitions?
Transitions will provide case management and volunteer services to individuals and their families, facing life-limiting illness, with an appropriate prognosis of one year or less without regard to race, creed, color, sex, age, national origin, religious background, physical disability, veteran status, medical treatment, or ability to pay.
Transitions will provide bereavement support as needed, which is crucial to enhancing one’s health and well being.
