Transitions
Transitions is a community service program designed to enhance the quality of life for adults, children and their families facing life-limiting illness. Services include emotional support, education and practical assistance. Based on specific needs, an individualized plan is formulated for clients and their families. There is no fee or charge for services provided to Transitions clients.

Hospice
The philosophy of hospice care is to provide comfort and pain management when cure is no longer an option.  Our goals are to support quality of life, to honor the wishes of our patients, to maintain the dignity of the individual, and to support the family through their grief process.

Our hospice care team collaborates closely with the patient's own physician to create a plan of care which is driven by the needs and wishes of the patient, family and loved ones.

Grief and Loss Counseling
Hospice of the Valley offers a broad range of loss-specific adult support groups, as well as age-specific loss groups for children and teens using art therapy and other expressive arts, facilitated by professional counselors. On a sliding scale fee, individual, family and group counseling available.
