We offer the Transitions program at no charge to enhance the quality of life for adults, children and their families who are facing a serious illness. Transitions offers emotional support, education on living with a serious illness and overall coordination to help you and your family find and utilize community resources and navigate the healthcare system.
During an individualized assessment, the Transitions program coordinator meets with you in person to better understand your needs and develop your own unique care plan. We offer a range of assistance such as coordinating your transportation, finding caregivers and respite for family members, and help with completing Advanced HealthCare Directives. In addition, the Transitions team offers an array of complementary and in-home services through the Hospice of the Valley volunteer team, including art, massage therapy, Reiki, Pet Companions, music, aromatherapy, help with meal preparation and so much more.
