What if I’m not ready?

If you’ve said to yourself, “I’m not ready for hospice care, but I could use some help,” turn to the Transitions program of Hospice of the Valley. We know that coping with a serious illness requires expert care, compassion and personal attention. We can help now even if you are continuing to seek curative treatment.

Hospice of the Valley offers the Transitions program to enhance the quality of life for adults, children and their families who are facing a serious and possibly incurable illness. It’s an innovative approach to ensure your goals and wishes are fulfilled. The program is offered at no charge to anyone with a prognosis of one year or less and includes emotional support, education and practical assistance. Our goal is to help you and your family find and utilize community resources, navigate the healthcare system and have the support you need along the way.

We can help:

  • Access and coordinate community services and resources
  • Offer a break for family and caregivers
  • Provide complementary services, such as art therapy, massage, Reiki and pet companions
  • Guide you in having discussions about your health concerns with doctors and other medical professionals
  • Facilitate your discussion about health concerns with your family
  • Educate you and your family about Advance Directives and help you complete Advance Directives forms
  • Complete errands, chores and food preparation
  • Coordinate transportation

Knowing all of your options for care and support empower you to make your own decisions and ensure your wishes are fulfilled. Even if you aren’t ready for hospice, call us at 408.559.5610 to learn more about our Transitions program. Services are provided at no cost to eligible residents of Santa Clara County. We come to you wherever you call home, whether that is a private residence, assisted living facility or nursing home.

Hospice of the Valley provides the Transitions program as a service to the community. The program is funded through the generosity of grateful families and compassionate community members who believe in the support and encouragement it offers individuals and families during challenging times.
