Hospice Nurse Case Manager
Hospice of the Valley is seeking RN Nurse Case Managers. Positions are Monday through Friday 30-40 hours per week. Salary dependent on experience and skill level. Benefits provided.

The Hospice Nurse Case Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

Hospice Nurse - LVN
Under Physician's orders, the hospice (LVN) nurse is responsible for identifying and coordinating patient/family care to provide support to the terminally ill patients and their families in their homes. Frequency of patient / family contacts will be at the discretion of the supervisory RN and her assessment of need, but will be at least weekly. The LVN endeavors to utilize her vocational nursing skills to provide comfort care and maximize the quality of life for the patients and families served.

Hospice Nurse On Call
Operating within the framework of policies and procedures, state and federal regulations and the patient's plan of care the on-call nurse is responsible for the management and care of Hospice of the Valley patients after business hours. The on-call nurse will be available by phone or pager to meet the needs of the patients, as assigned, from 5PM- 9:00AM Mondays through Fridays and for the 24 hour period beginning at 9AM on Saturdays and Sundays. The Hospice On-Call Nurse will endeavor to utilize their teaching, assessment, observation, and intervention skills to provide comfort care and maximize quality of life for the patients and families.

Transitions Program Coordinator
An exciting opportunity exists to further develop a dynamic community service program sponsored by Hospice of the Valley. The Transitions program is offered to individuals facing a life-limiting illness with a prognosis of one year or less.

Utilizing a case management model, valuing a psycho/social/spiritual approach, Transitions clients are provided expert referral and coordination to community resources, in addition to trained volunteer support. Hospice of the Valley is looking for a self-motivated Social Worker or experienced social services professional, preferably with case management, home health or hospice care experience. Program development skills are beneficial to lead this evolving program to its next phase. Working with our referral sources and promoting Transitions within Santa Clara County, as well as seeking program funding and maintaining a self-sustaining budget are key responsibilities of this position.
